2008 Applications

These are the same application forms that are available at Plaza TV & Appliance. They are available in both .pdf (Adobe Reader) and .swf (Macromedia Flash) formats. Simply download the forms, print them out, complete them and mail them - along with any other pertinent materials - to the address listed on the applications.

Miss West St. Paul 2007 candidates and their parents/guardians must complete the application agreement, application and the waiver of liability, or their applications will not be accepted.
